The amount of distortion with unders varies from person to person but if you work out a lot I am sure the Dr will recommend overs, if you can find someone who will perfom overs rather than unders even with little tissue I would certainly push for it. I have had both unders and overs, the distortion didn’t bother me so much in terms of how it looked but the feeling was unbearable, not to mention they cut the muscle so your upper body strength will take a blow, if you pole dance that will be a big problem for you. x
