Bad nipple and boob pain after having done for 2 years?!

So I got my boobs done in March 2016, they’ve been fine until now. My right one is hurting so much and especially my nipple, I think it’s underneath, I can’t wear no bra because even if something rubs it, it kills! Anyone else have this problem not sure what to do!
