Consultation help!

Hi everyone!
Just recieved a voice mail this afternoon from Mya which was just confirming my consultation on Monday they also mentioned have I filled in my medical form and to bring that too, did anyone else have this? Wasn’t aware of any forms to bring with me! Thanks xx

I filled all the forms out when I was there Hun I had ones through email that I had to lectronicaly sign tho xxx

Hope your consultation went well, they sent my forms through the MYA app which they will email you xx

Hi if you download the app (they should of text you the link) you have a to do list on there and it’s in there xx
