Doctor charging to sign letter?

Girls has anyone doctor charged them to sign the form? Mines just charged me £20 but I didn’t think you paid if a company had asked for it ?

Whys mine trying to charge me more !? Such a liberty when we’ve paid mya to do the surgery I thought you didn’t pay if it was requested from a clinic x
