Medical summary advice

Can someone help advise me with What information do we need to gather for the medical summary. I rang my gp this morning and asked and they wanted to know how many years worth of history do may need.. do they need correspondence of any referrals outside the doctors practice .

I just had to give them my nhs and enter my info on the medical summary I didn’t have to ask the docs anything. They then give me a letter to hand into docs x

I went into my doctors and said I needed a medical summary they called it a patient summary and they printed it off there and then… they did charge me £10 to do so though.. xx

@lian4 your lucky, I got told that i need a letter to say i am fit for surgery which was £10 and then they charged me £35 to print the summary #rip off GP 🙁 ;( xx

@LE my Doctors told me it would be £40!! £40 to press print lol! Xx

I just called my doc said I needed medical summary and covering letter they went right back to when I was like 3 years old! Didn’t have to pay and it was done in a couple of days x
