I’ve had my doctor fax over my full medical history.. That’s all been signed off and with Mya.
I had a mini suspected stroke a few years ago and my PC said I needed to get a hospital letter regarding that and get that faxed over. Anyway when I saw the nurse she didn’t mention it just said the doctors letter.

Has anybody else had to get a hospital note? As well as a doctors one? I’ve called Mya and they say do whatever the nurse said but in just thinking I don’t need my op cancelled on the day xx

Hello 🙂 I had to get a doctors note and hospital forms also due to having a gastroscopy and discovering I’ve got a hiatus hernia, chronic gastritis and ulcers. I think it’s best to get everything so it’s not left until last min. All of that was signed off by the surgeon and then my nurse asked for some more medications from my doctor about a certain medication. The nurse said its best to get everything you can rather than rushing about and it being too close to the surgery date. Maybe your doctors will be able to get your hospital files up if the hospital have sent it back to your doctors ? That’s how I got mine was a lot easier. Hope this helps x
