Overs or Unders??

So..my burning question is , overs or unders?
Who decides. The surgeon or client.
Is it true overs are less painful and heal quicker , what the pros and cons?
Why have unders if more pain ? Can anyone enlighten me as I’m very curious???? Thanks xx
