Registration form from hospital

I’ve just received a registration form from Ross hall hospital in Glasgow. What do I do with this?? It talks about medical insurance etc :S also do you getnyour admission time from this letter or from mya? X

I think MYA contact you,my PC contacted me,another member received that form,I think she said it was automated,nothing to worry about,but check with your PC to be sure.xx

I jst filled out the nxt of kin stuff an took it with me, but they didn’t need it in the end. I got my admission time bout a month before my op, think it was a letter from Ross Hall, u can always phone the hospital to find out x
